Thank you for your interest in Foundation Academy! Please be advised that we are no longer accepting new enrollments for the 2025-2026 school year.
If you are interested in enrolling your student for the 2026-2027 school year, please submit an inquiry or schedule a tour.
You will need an account in our Enrollment Portal to enroll online.
Required Documents
Enrollment is not complete until you have submitted the documentation required for enrollment at Ohio public schools.
- Proof of identity/guardianship for the student
- Proof of residency for the family
- Parent/legal guardian photo id
- Immunization/shot records for the student
