Voluntary Student Accident Insurance

Our school provides parents with the opportunity to purchase optional low-cost accident insurance for their students for the 2026–2027 school year. This coverage can help fill gaps in your primary health insurance, such as high deductibles or co-pays.

Coverage Options

You can choose from several levels of protection based on your student’s needs:

• 24-Hour-A-Day Plan: Worldwide, around-the-clock protection for covered accidents at home, at play, at school, on vacation and while playing most sports. Coverage continues through the summer until school reopens.
• School-Time Plan: Covers your student while school is in regular session, during school-sponsored and supervised activities, and while traveling directly between home and school.
• Optional Football Coverage: For students in grades 10–12 (including grade 9 if playing with grades 10–12) in interscholastic tackle football. Covers the 2026 season only, from the first official practice through the last official game, including playoffs.

2026–2027 Rates

One-time annual payment per student. Each plan is offered with a low or high benefit option.

Important Details

• Effective Dates: Coverage begins on the date your premium is received, but not before the opening day of school.
• Other Insurance: If you have other insurance, these plans can help offset its deductibles and coinsurance. If you have no other insurance, they provide basic coverage.
• Benefits: Up to $25,000 per injury for covered medical expenses that begin within 30 days of the accident and are incurred within 52 weeks of it.
• Term: Policies are non-renewable and cover the 2026–2027 school year. No refunds are available.

How to Enroll

1. Print the enrollment form on the last page of the 2026-27 Brochure & Enrollment Form and check the plan and options you want.
2. Make your check or money order payable to Rinehart, Walters, Danners & Assoc. Please do not send cash.
3. Mail the completed form and payment to:

Filing a Claim

Report any accident to the school office. Complete the Accident Claim Form and send it with itemized bills to Guarantee Trust Life Insurance Company, PO Box 1144, Glenview, IL 60025. Claims and bills must be received within 90 days.

The plan is administered by Student Protective Agency, Mount Vernon, OH, (800) 278-2544, and underwritten by Guarantee Trust Life Insurance Company, (800) 622-1993.
